For those interested in becoming a Functional / Integrative Medicine patient.
When you call our office to schedule your initial Functional Medicine consultation, our staff will send you a “Welcome” email confirming the date & time of your visit, the paperwork “due date”, and the “Document Open Password”, which you will need in order to open the intake paperwork packet. When you receive this email, please download the packet from our website, print it out (single-sided), and complete the forms, by hand, using BLUE ink. There is a section of the packet entitled “Health History Summary”; please type this portion, as you will be expected to share a great deal of information. As you go along, you may remember things, which you will then need to go back and put in…it’s just much easier to type it. The Functional Health packet is approximately 30 pages long, in .pdf format.
If you do not receive this email within roughly four hours of speaking with us, check your email’s “spam” filter (just in case the email went there). If it is not, please call our office immediately and our staff will attempt to re-send the information.
It is extremely important that you answer every question; fill in every blank, and check every appropriate box. Please refrain from using the phrase “I don’t know”; take your time, think it over, and give your best answer. The more detailed information that we can gather about you, the more complete and effective your care will be.
This intake packet must be returned to our office, in person, no later than 2-weeks before your initial appointment. When you are ready to submit your packet, please call our office to schedule a short 10-15 minute no-charge visit so we can set aside a little bit of time to check over your paperwork, to ensure that everything is in order.
IMPORTANT NOTE…If we do not receive your packet on time, your initial appointment may have to be rescheduled in order to allow the proper and necessary time for the doctors to review your information.

If your visit is due to a recent Motor Vehicle Accident, please fill out the Motor Vehicle Accident Form instead.
If you are seeking Chiropractic Manipulative Therapy (Adjustments), Acupuncture, Medical Massage Therapy, or Ancillary Services due to injuries suffered as a result of a Motor Vehicle Accident, and wish to utilize the Personal Injury Protection (PIP) coverage of your Automotive Insurance policy, please download and complete the PIP Information Form, and return it to our office BEFORE your first appointment, so that we may verify your coverage.
If you are interested in beginning Chiropractic-based Acupuncture and/or Traditional Chinese (Asian) Medicine, please download both of the TCM Intake and Evaluation packets, print them out (single-sided), and complete them in BLUE ink. Patients receiving Medical Massage Therapy as directed by their SAFIH doctor as part of their care plan, do not need to fill out additional massage therapy intake forms.
Clients who would like to receive massage therapy at our clinic, may schedule with our therapists even if not under the care of one of our doctors. Please understand that as we are a doctor’s office, our therapists are highly-trained, and very experienced with goal-oriented problem-solving techniques which may differ a bit from the typical massage studio or spa setting.
Please download the Massage Therapy Intake Form, print it out (single-sided), and complete it in BLUE ink. This paperwork should be completed by the start of your initial visit; this is to allow the therapist a chance to review your information, so that the session can be tailored to your specific needs. Please Note: A Time of Service (TOS) discounted rate may be applied to services which are paid for “in full” on (or before) the date the service is provided. Any services not paid “in full” on/by the date the service is performed must be charged at full Usual and Customary Rates (UCR), as dictated by the Insurance Industry and by Federal Law.
We currently accept auto accident Personal Injury Protection “PIP” claims (from most carriers) for Chiropractic Manipulative Therapy,, Chiropractic Acupuncture, Medical Massage Therapy, and Ancillary Therapies such as: Estim/TENS, Hot Packs, Segmental Distraction (roller table), etc. We will do all of the necessary insurance filing on your behalf.
We are otherwise an “out of pocket” clinic, which means you pay as you go. For those wishing to use Major Medical Health Insurance benefits (BC/BS, UHC, Humana, Cigna, Aetna, etc) for our services, we can gladly provide you with a “Superbill”, which is an itemized invoice that contains appropriate insurance coding information necessary for reimbursement* from your insurance company. As we do not file this type of insurance billing, it would be solely your responsibility to file with your insurance company if you wish to be reimbursed for your out-of-pocket expenses.

An option which many of our patients find helpful, is to explore the various options at their local bank or credit union. Many financial institutions offer convenient, no-collateral signature loans, which can be used to pay for various types of healthcare that insurance does not cover, such as orthodonture or Lasik eye surgery. These advances can be used to cover the costs of our services. Applying is generally fast and easy, and the terms are generally fair. You can stay within your chosen budget without sacrificing the healthcare you need!
Welcome! We have a few housekeeping items that we need to share, so everyone will be on the same page, and so that everyone can have an enjoyable, relaxing, and healthy experience when visiting our office.
Punctuality: We ask that you arrive on time for every appointment. We understand that life happens; and other things may need to occupy your time. If you know you’re going to be late, please call and let us know, so that we don’t worry and wonder what happened to you.
Perfume & Cologne: Please refrain from wearing any scents or fragrances into the clinic. We work with many patients who are very sensitive to chemicals, who have allergies, and/or who have overactive immune systems. We don’t want people to fear or dread coming into the clinic; they’re supposed to feel better when they come in, not worse!
Payment: Unless a Third-Party Payor Agreement (or other arrangement) is in effect, all services, products, lab testing, and interpretive work must be paid for at the time of service/ordering. All of the prices quoted in our Financial Agreements are Time Of Service (TOS) discounted rates. Pursuant to insurance rules and Federal Law, for any balance not paid for in-full at the time of service, we are required to charge our established Usual and Customary Rates, which are typically much higher than TOS rates. We are legally bound by this, and can offer no leniency.
Short-term Illness: If you are sick with a regular cold, flu, or other minor illness, but can otherwise make it to the clinic, please do come in for your CMT and/or acupuncture treatment! (Please, however, reschedule any ancillary therapy for another day, as it may make you feel much worse before you feel better). Please do stay home if you have a contagious disease such as bronchitis, pneumonia, or any other condition that can be transmitted. Please do seek medical treatment for such issues, and please DO return when you are no longer contagious! We can help you get better, faster.
HIPAA: Please rest assured that we strive to maintain HIPAA compliance, and that your Personal Health Information (PHI) will not be shared with anyone else (even your friends or family members) without your written permission. The only people we do share health information with, are other relevant practitioners in the clinic, or with our colleagues, in order to obtain further expert advice on how best to serve you; or, whereas required by applicable law.
In the interests of protecting privacy and providing the best service possible; we ask that in order to discuss healthcare-related issues including information, advice, or recommendations, please schedule an appointment to see your doctor. This allows us to set aside appropriate time for you in a private setting, We do not give out any official information, advice, or recommendations over the phone*, via email*, in the front lobby, or anywhere else outside of our offices. We appreciate your understanding and cooperation.
*Scheduled Phone appointments are available to established Functional Health patients under certain circumstances. Email discussions and Skype appointments may be offered in the near future.
Although we make every effort to confirm appointments, by phone or via automated text system, the day before a scheduled visit, it is ultimately the responsibility of the patient/client to remember and keep their appointments.
In order for us to secure any appointment, we require that every patient/client keep a current credit card on file with our office. This information will be kept secure as part of your office records, and will only be used to process payments, or to comply with office policies. The information will otherwise be released only whereas required by applicable law.
All CMT, acupuncture, and ancillary services/therapies require a 2-hour notice in order to change, re-schedule, or cancel any appointment. If notice is not received in time, or an appointment is missed, the patient/client will be responsible for the Full Cost of the appointment.
All Functional Health Initial Consultations require a 72-hour notice in order to change, re-schedule, or cancel the appointment. If notice is not received in time, or an appointment is missed, the patient will be responsible for the Full Cost of the visit, as we block off that entire portion of time, and can not schedule any other patients.
All Functional Health “regular” appointments require a 2-hour notice in order to change, re-schedule, or cancel the appointment. If notice is not received in time, or an appointment is missed, the patient will be responsible for a $90 Cancellation Fee.
Established Functional Health patients canceling a regular appointment on short notice may avoid the Cancellation Fee as long as the visit is made up within the same week, during available office hours*. If the make-up appointment is canceled with proper notice, the Cancellation Fee from the first appointment shall apply. If the make-up appointment is missed or canceled without proper notice, the Cancellation Fee’s for BOTH appointments shall apply.
We certainly understand that life happens; appointment changes, re-scheduling, and cancellations due to sudden illness or serious emergencies will be evaluated on a case-by-case basis.
*If there are no “available” office hours, or if the patient is unable to schedule within “available” office hours, appropriate Cancellation Fee’s shall apply.
