A Functional Medicine Look at Fatigue That Lingers After Levothyroxine

“My labs are normal, but I still feel exhausted.”
If you’ve been diagnosed with hypothyroidism, you were probably prescribed levothyroxine (or another T4-only thyroid medication). And for some, that’s enough.
But if you’re still tired, foggy, cold, or struggling to lose weight, you’re not alone—and it doesn’t mean you’re imagining things.

The Limits of Standard Conventional Thyroid Treatment
Most conventional doctors test TSH (thyroid stimulating hormone) and, if it’s high, prescribe T4 replacement (like Synthroid or levothyroxine). But TSH is just one part of the picture.
In Functional Medicine, we often see patients with “normal labs” who are still suffering. Why? Because:
- T4 must convert into T3, the active form of thyroid hormone
- That conversion can be blocked by stress, inflammation, gut issues, or nutrient deficiencies
- Some people don’t respond well to synthetic T4-only medication
- People can also develop thyroid hormone resistance, where the cells simply don’t recognize hormone therapy
- The autoimmune component (Hashimoto’s) often goes unaddressed
- Autoimmune conditions have drivers that conventional providers aren’t even aware of
Key Reasons You Might Still Feel Fatigued
1. Poor T4-to-T3 Conversion
If your body isn’t converting T4 to the active T3 form efficiently, it’s like owning a car with a full tank of gas—but no key.
Common culprits:
- Inflammation
- Low selenium or zinc
- Stress hormone imbalance (high or low cortisol)
2. Unrecognized Hashimoto’s Autoimmunity
Over 90% of hypothyroidism in the U.S. is autoimmune. That means your immune system is attacking your thyroid, even if your TSH looks “normal” right now.
Without addressing the underlying immune dysregulation, symptoms can persist—even on medication.
3. Adrenal & Mitochondrial Dysfunction
Fatigue can be driven by your stress response system (HPA axis) or by the mitochondria, which generate your cellular energy.
Thyroid meds don’t fix those issues—so your energy stays stuck.
4. Nutrient Depletion
Your thyroid and energy systems depend on:
- Iron and ferritin
- Vitamin D
- B12
- Magnesium
- CoQ10
Many patients are unknowingly deficient, even with a good diet.
